Then download this order form and fill out the form with your complete name, mailing address and e-mail address. Mail your completed order form, the original cash register/purchase receipt(s) dated between 11/8/09 and 12/7/09 with the participating products circled and the corresponding UPC’s from five (5) participating products to:
